Information
to Bring To an Office Visit:
1
Please bring your insurance cards, list of medications you are currently
taking with dosage, any medical records, or test results to your visit
with us.
2. If your insurance
requires a referral, please come prepared with your Primary Care Physician
Referral and be prepared to pay your copayment as required by your insurance
company.
3. If you are a new
patient to our office, please arrive 10-15 minutes early to complete the
paperwork. If an existing patient, please arrive 5 minutes early.
How
do I reach my physician after hours?
After the office closes
our telephones are answered by a voice mail system. Call the office and
page the on-call physician. For routine questions regarding billing, medications,
etc., please call between 9:00-12:30, and 1:30-5:00. All appointments
with the doctors are by appointment only.
How
do I get my prescription refilled?
Always call your pharmacy first. Please
remember that many diseases have similar symptoms, therefore, most requests
for treatment require examination. Please do not ask for telephone prescriptions
other than refills unless the doctor has recently examined you. Phone
the pharmacy and ask them to contact our office for the refill.
How
do I get copies of my medical records?
We need a signed consent
form from you to release your medical records. The release must include
your name, the date, and your doctor's name and the address where they
are to receive the records.
X-rays
and Lab Work
Most laboratory and
x-ray tests will be performed by the outpatient facilities at Sonora Regional Medical Center and Quest.
Finacial
Policy
Our fees are comparable to specialty surgeons in the Mother Lode area.
We request that you pay for office services the same day. We will give
you the necessary forms to bill your insurance. Please do not ask the
Office Manager to bill your insurance company except in the instance of
surgery. Payment of fees is the responsibility and obligation of the patient.
Recovery of insurance payments is between you and the insurance company.
Payment in
full at time of service - Visa, Mastercard, check, and cash
are accepted.
HMO/PPO:
Co-pays due at time of service.
Private Insurance:
We bill only those companies that we are contracted with except for hospital
surgery or office procedures over $150.00. We do not bill secondary insurance
unless we are contracted. We will not become involved in disputes between
you and your insurance company regarding deductibles, co-pays, etc., other
than to supply factual information as necessary. We will provide necessary
billing papers to you to file your insurance claims if we are not doing
the billing. Regardless of insurance coverage - you are ultimately responsible
for the balance on your account. It is patient responsibility to contact
the insurance company and pursue late reimbursement and to make financial
arrangements with our office if the insurance payment is delayed.
Medicare/
Workers Comp: It is the patients responsibility to provide billing information,
claim number, case number, billing address, and employer information at
the time of service. If balance is left on an account for over 90 days,
the account will be turned over for collection unless arrangements are
made with the office manager in advance.
Insurance
Affiliates: We bill most health insurance plans including: Medicare, Blue Cross, Blue Shield, First Health and Interplan. |
